  5. Certificate from Headmaster as per attached format;

• For Post-Matric Scholarship for the students of Tea Tribes Community of Assam

  List of documents to be uploaded:

  1. Original copy of Birth Certificate/ HSLC Admit / HSLC Pass Certificate as age proof;
  2. Copy of Mark-sheet of last examination passed to be uploaded;
  3. Admission/fee receipt of the course undergoing;
  4. Undertaking on family income as per attached format;
  5. Original copy of caste certificate;
  6. First page of applicant’s bank passbook.

• For Simon Sing Horo Special Scholarship Scheme

• For Grants to Women Self Help Groups (SHGs)from Tea Tribes Community of Assam

  List of documents to be uploaded:

  1. Original copy of up to date Registration Certificate;
  2. Names of all the members;
  3. Recent group photograph of all the SHG members;
  4. Undertaking affirming all members belong to Tea Tribes community as per attached format;
  5. Bank statement of last 6 months;
  6. First page of applicant’s bank passbook.
